Horixons Inc. provides compassionate, reliable Homemaker Personal Care (HPC) services for individuals with Diverse Abilities across Ohio, supporting participants enrolled in Ohio Medicaid Waiver programs and services overseen by the Ohio Department of Diverse Abilities (DoDD). Our person-centered services are designed to help individuals live safely, independently, and with dignity in their own homes and communities.

Our trained, background-checked direct support professionals assist with activities of daily living, including personal care, hygiene, meal preparation, mobility support, and household tasks, in accordance with each individual’s Individual Service Plan (ISP). Horixons Inc. works closely with families, Service and Support Administrators (SSAs), and care teams to ensure compliance with DoDD rules, continuity of care, and consistent, high-quality service delivery under Ohio’s Medicaid waiver system.
