Our mission is to empower clients to live safely, independently, and with dignity. We offer personalized support including personal care, meal preparation, light housekeeping, transportation, behavioral guidance, and skill-building programs. By creating customized care plans, Horixons ensures every client receives the consistent, professional support they need to thrive at home and in the community.

We serve Mahoning and Trumbull counties. We work with your insurance and local county support programs to serve your needs.

What services do you provide?

We offer a range of services including personal care, homemaker companionship, 24/7 support, transportation to day programs and work, and specialized care for advanced needs. Each plan can be customized to meet individual client needs. We serve clients on Department of Aging, Mycare Ohio and Department of Developmental Disabilities plans.

Where can I use your Transportation Services?

We provide safe and reliable transportation services for individuals with developmental disabilities, including rides to day programs, schools, workshops, and place of employment. In addition, we offer Homemaker Personal Care (HPC) transportation to support access to essential services and full participation in community life.

Are you caregivers certified?

Yes, all our caregivers are certified, trained, and experienced in elder care and care for individuals with developmental disabilities. They undergo continuous training to provide the highest quality of care with compassion and expertise.

How do I choose the right care plan?

Our team and Registered Nurse will consult with you, your case manager, your social services administrator, and family to assess your specific needs and recommend a care plan that best fits. We will then assign most suitable primary and secondary care provider to you. We're here to answer any questions and make adjustments as necessary.

Can I make changes to my care plan later?

Absolutely. By default we re-visit the care plan every 60 days. We understand that needs can change over time. We work with families to make any necessary adjustments to ensure the highest level of comfort and support as often as needed.

Is 24/7 care available?

Yes, we offer 24/7 support for clients who require around-the-clock care and monitoring. Our caregivers are available at any time to provide assistance and ensure safety and comfort.

How do you ensure client safety?

We follow strict protocols to ensure safety at all times. All care provider staff go through thorough criminal background checks and drug screening.

How do you ensure client privacy?

We follow strict protocols to protect client privacy. Our caregivers are trained in confidentiality, and we use secure systems to manage client information. We follow same protocols and policy as your doctors would to ensure privacy of your information.

Do you hire family members?

Yes, We believe that care delivered by natural supports is among the most effective forms of care. When allowed by law and program guidelines, Horixons Inc. encourages natural supports to become part of our team to help care for their loved ones.
